Answer the questions on your past or present medical
history with a YES or NO. If you are not sure, answer YES. If any of
these items apply, you must consult with a
physician prior to participating in any in-water scuba training. This means that you must have your physician review the complete form and sign the form indicating either that he or she finds no medical conditions that I consider incompatible with diving or that they are unable to recommend you for diving.
If you are completing your Open Water certification dives with Scuba Shack, we require that all students have the medical statement signed by their physician indicating that he or she finds no medical conditions that they consider incompatible with diving. General Information
- A positive response to a question does not necessarily
disqualify you from diving. A positive response means that there is a
preexisting condition that may affect your safety while diving and you
must seek the advice of your physician prior to engaging in dive
activities.
- The original form is kept on file with your student records and is valid for one year. You may use the same form for continuing education during year as long as there is no change in your status.
- If you are taking a continuing education class and answer NO to all questions, it is not required that you have your physician sign your medical statement.
- For professional-level certifications, Scuba Shack and all certification agencies require that the medical statement be signed by the candiates physician indicating that he or she finds no medical conditions that they consider incompatible with diving.
